If a person is elderly or unable to do manual labor, you might purchase a gift certificate to paint a room in their house, fix or repair broken items, etc. This work could be done by you or you can pay to hire a reputable repair man. This gift would "keep on giving" because the repairs will last forever.
Based on their financial situation, a certificate for some home cooked meals is an unique gift. You can purchase these, make them yourself, or have a group contribute to the effort. This is a gift that will be appreciated for many days.
